New recommendations for Low Back Pain

The National Institute for Care and Excellence (NICE) recommends exercise as a first step in managing low back pain, and advises people with low back pain to exercise:

  • In a group supervised exercise programme, in a group of up to 10 people
  • One-to-one supervised exercise programme may be offered if a group programme is not suitable for a particular person
  • Mind-body exercise, which includes Pilates exercise.

How does this work?

Pilates exercise improves body awareness and promotes optimal movement, which “is thought to manage CLBP (chronic low back pain) by activating the deep spinal and abdominal muscles while promoting greater spinal stabilisation. This specific stabilisation training has been shown to improve pain, reduce disability, and increase symmetry of the lumbar multifidi at the vertebral level in healthcare workers with CLBP” (Stieglitz et al. 2015)

How do we do this?

Not all Pilates classes are suitable for people with non-specific low back pain, or with chronic low back pain. For example, dynamic Pilates classes tend to work on larger, more superficial, power producing muscles of the spine, whereas our Pilates approach aims to initially recruit the deep stabilising muscles of the spine, then layer the superficial ones.

The original classical Pilates method promotes a flat back, whereas, currently accepted practices in sports rehabilitation believes that being able to stabilise the spine in a neutral zone is key to recovery.
We do this by retraining the deep transverses abdominis, multifidus preferentially with exercises tailored to people with low back pain. We teach you to stabilise the spine with lower loads, closed chain, slower movements and build up to recruiting larger muscle groups gradually.
